Double incision mastectomy (with free nipple graft/liposuction) is usually carried out as a day case procedure and is performed under general anaesthetic. The surgical procedure is designed to result in a flat masculine aesthetic by removing breast tissue and excess skin.
The surgery involves the following key steps:
- Incision Placement: Incisions will be made to remove breast tissue and create a masculine chest contour. The double incision approach allows for optimal tissue removal and reshaping.
- Liposuction: A number of small incisions are made adjacent to the area where the fat is to be removed. A mixture of local anaesthetic, adrenaline, and saline are introduced to help loosen the fat, gentle liposuction is then performed through a small cannula. *optional*.
-
Nipple Grafting: The nipples are removed and then repositioned to achieve a more masculine appearance. Free nipple grafting involves reattaching the nipples after removal, allowing for precise placement. *optional*
-
Closure: Once the necessary adjustments have been made, incisions are closed with sutures. Drains will be placed to prevent fluid buildup during the initial stages of recovery. The final shape of the incision will produce linear scars that will extend from the midline toward the armpit. You will also have a circular scar around the nipple graft.
NB: Double incision mastectomy is available with and without nipple graft and with and without liposuction.

BUTTONHOLE TECHNIQUE TOP SURGERY
This technique comes under the inferior pedicle category of top surgery procedures. This means that the nipple remains attached to a layer of tissue with the intention of retaining full nipple sensitivity, though this is not guaranteed. Key considerations are as follows: the procedure results in less flatness, the repositioning of the areola is restricted, the muscle definition is less achievable
Recommended for people with middle-sized chests and bodies.
Typical duration: 2 – 4 hours
Is nipple sensation preserved: in most cases.
Is nipple repositioned: Yes without graft
Scarring: Significant.

Buttonhole technique top surgery or mastectomy is usually carried out as a day case procedure and is performed under general anaesthetic. Similar to Double Incision Mastectomy, two horizontal incisions are made at the top and bottom of the breast. Instead of using free nipple grafts (as with Double Incision) the nipple and areola are re-sized and left attached to nerves and blood supply via a thin dermal pedicle (or flap of tissue), which in most cases can preserve sensation. It is important to note that the presence of this layer of tissue (pedicle) under the skin might not give the same flatness as the double incision mastectomy.
The surgery involves the following key steps:
- Incision Placement: two horizontal incisions are made at the top and bottom of the breast with a third incision around the areola to bring the size into line with what would more typically be found on a male chest. A final incision is then made to the lower tissue that will remain behind (also known as the inferior pedicle) The pedicle with nipple attached is then peeled forward and the upper layer of skin removed. The top and bottom incisions are then pulled together and the flap with the attached nipple is placed under the upper section of the chest.
- Nipples: A small incision is then made and the nipple and areola are brought from under the skin through this opening—similar to threading a button through a buttonhole—and stitched into place
-
Liposuction: A number of small incisions are made adjacent to the area where the fat is to be removed. A mixture of local anaesthetic, adrenaline, and saline are introduced to help loosen the fat, gentle liposuction is then performed through a small cannula. *optional*.
- Closure: Once the necessary adjustments have been made, incisions are closed with sutures. Drains will be placed to prevent fluid buildup during the initial stages of recovery. The final shape of the incision will produce linear scars that will extend from the midline toward the armpit. You will also have a circular scar around the nipple graft.
NB: Buttonhole mastectomy is available with and without nipple graft and with and without liposuction.
BATWING MASTECTOMY or FISHMOUT INCISION TOP SURGERY
This method results in a scar which radiates out from the nipple and is suitable for those with medium sized chests.
Typical duration: 2 – 3 hours
Is nipple sensation preserved: In some cases, yes.
Is nipple repositioned: There is some minor adjustment on position.
Scarring: The resulting scar pattern resembles a batwing, hence the name.
For more information on Batwing click here.
Batwing mastectomy is usually performed as a day case procedure, under general anaesthetic. The procedure is an extended periareolar mastectomy. It is recommended for individuals with a medium sized chest and delivers good results in those with excess skin, reduced skin elasticity, and mild drooping.
The surgery involves the following key steps:
- Incision Placement: During the procedure, a doughnut of skin is removed around the circumference of the areola, along with two triangles of skin on either side in order to address the skin excess, creating the distinctive batwing-shaped scar.
- Mastectomy: Breast tissue is then removed through this circular area of removed skin and any liposuction or contouring is performed.
-
Closure: The edges of the two incisions are brought together. Once the necessary adjustments have been made, incisions are closed with sutures. Drains will be placed to prevent fluid buildup during the initial stages of recovery. The horizontal scar aligns with the areola, and the areola may be reduced in size if necessary.
NB: Additional liposuction may be carried out as required.

Is nipple repositioned: Some minor repositioning is possible.
Scarring: Minimal.

Periareolar mastectomy is usually performed as a day case procedure, under general anaesthetic. The surgical procedure is designed to result in a flat masculine aesthetic by removing breast tissue through the areola (pigmented skin around the nipple).
The surgery involves the following key steps:
-
Incision Placement:A circular incision is made around the border of the areola followed by a larger circular incision around the first one.
-
Mastectomy:Breast tissue is then removed through this circular area of removed skin and any liposuction or contouring is performed.

Patients with a very small chest and tight skin may qualify for this chest surgery procedure. With this technique, no extra skin is removed. Keyhole mastectomy is usually performed as a day case procedure, under general anaesthetic.
The surgery involves the following key steps:
-
Incision Placement: A small incision is placed along the bottom border of the areola.
-
Liposuction: Any necessary liposuction and breast tissue removal is performed.
- Closure: The edges of the small incision are brought together and closed with sutures. Drains will be placed to prevent fluid buildup during the initial stages of recovery and to facilitate the attachment of the skin to the muscle.
NB: While the areola can be resized with this technique, the nipple is not detached and therefore cannot be moved to a new position.

LIPOSUCTION AND BODY CONTOURING
Liposuction is a type of fat removal procedure used in plastic surgery in order to improve the contour of an area, reduce deposits of fat, and tighten the skin (to a small degree). The procedure is usually performed as a day case procedure, under general anaesthetic or under sedation, and is offered as an add-on to top surgery in order to create a more natural overall appearance.

Liposuction can be used as part of the top surgery process in order to create a more natural appearance between the chest area and the rest of the body. The aim is not to remove all the fat from a particular area, but to remove excess fat from the area in order to create a more natural look.
